I understand that readmission is not automatic or guaranteed. I understand that my previous academic work information I have submitted with my petition, and any other pertinent information, will be used in making the decision. I understand that readmission and reinstatement may require meeting certain conditions. These will be determined at the time my petition for readmission is reviewed by the Admissions and Academic Standing Committee.

I understand that the entire readmission petition must be completed, submitted and received by the date and time specified on the suspension letter or it will not be considered. This information must be submitted to the Admissions Office. I understand that I have the option to appear before the committee and if I choose to do so, I will inform them by the date and time specified on the suspension letter.

I agree to allow the Admissions and Academic Standing Committee to retrieve any and all pertinent information so that they may make the best informed decision on my behalf.

I certify that the above information is true and correct to the best of my knowledge. Any student found guilty of NONDISCLOSURE or MISREPRESENTATION in filling out the Petition for Readmission from Academic Suspension is subject to disciplinary action and possible permanent dismissal from the University.
